TTS abstraction (multi-backend)

Unified TTS interface supporting ElevenLabs (cloud), OpenAI-compatible TTS servers (e.g. qwen3-tts-server with streaming PCM), and local TTS sidecars. Streaming vs buffered synthesis is a first-class option.
